Voluntary Life and AD&D will continue to be provided by Guardian.
Open Enrollment Guidelines:

Employees currently enrolled can increase their benefit amount up to $50,000 or up to the Guarantee Issue (GI) without medical question, known as Evidence of Insurability (EOI).

If you wish to increase your coverage by greater than $50,000 or above the GI, you must complete the required EOI form.

Employees currently enrolled with spouse coverage, can increase their spousal coverage by any amount, an EOI must be completed.

Employees who previously declined coverage for themselves and/or their dependents, but want to elect coverage during open enrollment, will be required to submit an EOI for any amount being elected.
Please note: any request to increase coverage that exceeds the Guaranteed Issue (GI) amount, will require and EOI.
If you are a new hire you can elect, in $10,000 increments to a maximum of $250,000.
You are eligible to elect up to $150,000 without medical question, known as Evidence of Insurability (EOI).
